We've decided that the unique indexes on k values should be case sensitive. Since this is a (non-rails) db-setup option, we should have a test to ensure the db is correctly set up (c.f. utf8 handling).

http://forums.mysql.com/read.php?103,156527,198794#msg-198794 seems that it will make all the other indexes case sensitive too.

MySQL-specific database setup options aren't relevant any more.
