the river is called "Neckar" but due to the island the label is put on the land not on the river.

I think this is a tagging problem, not a render issue. There should be no name tag on the waterway=riverbank way, only on the waterway=river way in the center of the river. There are more tagging problems (not directly related to this ticket): The big island is not part of the multipolygon relation as it should be (plus place=island and natural=land tags should be removed and riverbank tag should be added). The waterway=river is a closed way around the big island, it should be just one way in the main riverbed north of the island and pointing in the direction of the river flow. The layer tags on the riverbank, river and island ways should be removed.

OK, thank you, I corrected some of the issues but kept the name for the riverbank (because it should be identificable with name).
