A bridge crossing another bridge does not render correctly.
Two example cases (which may be different problems):

The A14 is layer 2 (both parts), the B road is layer 1 and the railway is implicitly layer 0. The casement on the A14 forms a cross with the B road (both times), rather than continuing across the bridge cutting across the B road (for reference, osmarender does this correctly)

The river(bank) is layer 0 implicitly, the cycleway is a bridge out over the river on layer 1 and the railway crosses the cycleway and the river on layer 2. But the cycleway is rendered over the railway. (The tagging is correct - here's a picture of the situation: http://cambridge.cyclestreets.net/location/10368/).
