The current mapnik style seems to render both ways and areas tagged man_made=pier if they jut out from a coastline, but only ways render when the pier is over waterway=riverbank.
Example of this can be seen in Seattle, WA, USA:

The area piers in that view aren't rendered by mapnik, but are by osmarender.

The area-piers are actually drawn, but then the water gets drawn on top. I have a fix ready, which will become active when it's deployed to the tile server.

way-piers a little up the river are rendered:

Both way-piers and area-piers render fine when the water is off a natural=coastline.
