If you start to draw a segment from a node, but do not complete it by dragging to another node, the applet creates a segment with the start and end point at the same node.

just like to add that this really should be prevented in the API as well as the editing applications. It causes problems with Thomas Walraet's JOSM plugin to automatically create ways, which interprets this madness as a junction.
