I'm not sure how hard this is. However, the 5 counties of New York City each correspond with one borough. Nominatim lookups should return the name of the borough as well as the county for addresses in that area. Also, free form address queries should try to match against borough name.

This really needs to be fixed in the OSM data somehow. Either add the borough name as an alternative name to the county boundaries or add an additional boundary=administrative for the borough. The first will resolve the search problem, only the second solution will ensure that both, borough and county show up in the address.
