This trac needs an extra state or something and an associated report so that people with commit rights can look for these patches (with an RSS feed) and apply them quickly. Sometimes a patch is offered but it doesn't get committed for months because no one notices it.

Why not have a rule that you have a summary of [patch] if the ticket contains a patch, then the following custom search can be used:
http://trac.openstreetmap.org/query?status=new&status=assigned&status=reopened&summary=%7E%5BPATCH%5D&order=priority

Which means that the following RSS (as linked at the bottom of the above page) can be used:
http://trac.openstreetmap.org/query?status=new&status=assigned&status=reopened&format=rss&summary=%7E%5BPATCH%5D&order=priority

This doesn't seem to have been a major issue, and I would be the main user of it.
