Hi

The top result for Cape Town South Africa is being displayed in a foreign language "Kapsk mesto" and should be displayed as "Cape Town"

"Kapsk mesto" is noted as name:cs, the Czech translation, which is correct according to Wikipedia. You probably have Czech somewhere in your language preferences. Nominatim always shows localized translations of place names.
